COS members in good standing may register online for the regular monthly conferences. Your pre-registration is necessary so that we can accurately plan for the dinner and a pleasant experience for all in attendance. To register a guest or if you owe a fee, use the PDF form. You can fill this out on your computer and then print it.

For a MapQuest map, CLICK HERE Previous Meeting CME Claim Forms Beginning in the 2012/2013 academic year, CME credit certificates will be mailed automatically to practicing physicians who sign in at the meeting. Please note that if you fail to sign the attendance sheet, we will be unable to verify your attendance in order to award credit.
Resident Case Presentations at Monthly Conferences Beginning in 2009, COS initiated a series of case presentations & discussions featuring residents from the various training programs in Chicago. This addition to the program has resulted in an interesting educational element for the evening and has been well-received by the membership. Chicago Ophthalmic Calendar
The COS works hard to keep track of conferences, meetings and events that will be of interest to the ophthalmic community in Chicago. Included are local meetings, national conferences of significance, and some international meetings.
We depend on you to help keep this information current. If you have an event to submit, contact the COS administrative office by sending an email to: [email protected]. Be prepared to provide the name of the event, the location (including address), a contact person with phone and email, the target audience and whether the meeting has CME credit. Also include a link to any online information for your meeting/event.
